BAC Grants Recipients
Follow the links on this page to take you to Submittable to complete each form. Full instructions for each report can be found within the Submittable form.
For final reports, please have on hand:
- Attendance numbers and demographic statistics
- Receipts for all BAC funds spent
- Budget with all tabs, including final report tab, complete
- Media documentation of the project
Recipients are responsible for noting their final report due date and submitting the report by or before that date. Recipients who do not properly report on their grant(s) become ineligible to apply to another BAC grant.
2020 Community Arts Grants Final Reports
All 2020 BAC grantees are required to submit a final report to BAC within 30 days of the project end date stated on their contract, or January 15, 2021, whichever is sooner.
Community Arts Grants include Brooklyn Arts Fund, Local Arts Support and the Charlene Victor & Ella J. Weiss Cultural Entrepreneur Fund.
For Brooklyn Arts Fund recipients, the 20% payment for your grant is dispersed after your final report is reviewed.
Recipients who do not submit final reports become ineligible to apply again.

2020 Project Extension Request Form - Community Arts Grants
If your project timeline has significantly changed, you may be required to submit a project extension request. After review and approval by staff, we will send you a contract addendum, authorizing the use of funds beyond the originally contracted date.
* Extensions cannot be granted past March 31st, 2021.
* Extensions only apply to Brooklyn Arts Fund, Local Arts Support, and the Charlene Victor & Ella J. Weiss Cultural Entrepreneur Fund grants. Destination>Brooklyn grantees are not eligible for extensions.
