CONTRACT AND W-9

In mid-February, we will email you a BAC Grantee contract specifying the terms of your funded project along with a W-9. The contract and W-9 must be signed by the artist or an organization’s authorized representative. If you are funded through a fiscal sponsor, an authorized representative of their organization must sign as well.

Original grant contracts and W-9s must be signed and returned to BAC's office by the deadline in order to receive funding. Your copies will be included with your award check.
